Welcome to our practice. We have been serving residents of Medina County since 1987. Currently our group has 4 physicians & 2 nurse practitioners.
Effective 1/1/2022, our practice is participating in Medicare accountable care organization (ACO). Please find details in the attached form on the right side of the screen in the box "Attached document and forms"
We have a fully functional Electronic Medical Record that help us track health care much more efficiently.
You have free access to the patient portal where you or any one you authorize can see your test results & also communicate with us electronically for non urgent communications.
We are providing excellent continuity of care by seeing you both in the office and also if admitted to Medina Hospital, nursing facilities, including assisted living or group homes in this area.
Our group has physicians with board certification in internal medicine, Geriatric medicine (specialized care for the aging usually above the age of 75), Pulmonary medicine ( Lung diseases) & Sleep medicine.
To get the best care we recommend that you follow certain steps:
1. Always come prepared to your visit with your questions & your needs written.
2. Prior to any healthcare encounter ( office, ED)- you should have a complete list of your medicine. Please also indicate those that need to be refilled.
A good practice is to put all your medicine in a bag & bring them to the visit. That way the name of medicines and doses can be verified.
This should include medicine prescribed by any physician. It is good practice to keep updated list of medicine with doses in your purse or keep a picture on your phone.
3. If you have diabetes you should always bring your home glucose record for review
4. If you have change in your information including your health insurance, your contact information or any change in your family history, please update it on portal or when you come for visit.
5. If you have high blood pressure, you should always check your blood pressure at home & bring your machine & readings to office for review.
6. You have link on our site to get a free blank living will & durable power of attorney forms. Please print these forms & have them notorized or witnessed by two unrelated witnesses.
7. When you see any other physician, or go to emergency room always notify them about us taking care of you so that they can send result of your visit to us .This is very important to provide you comprehensive care. You should bring the signed form to our office to be scanned in your medical record.
